How to apply
Initial 10 hours counselling
You may complete and send your application for counselling online. Please remember to save your form before pressing the submit button.
Use of this form requires that Adobe Reader (8.01 or above) be installed on your computer. The free Adobe Reader software can be downloaded from the Adobe website.
At the end of the form you are asked to press a SUBMIT button which will send the application to Victim Services. If the submission process has been successful you will see on your screen a copy of the form with an enquiry number near the top. See sample image of the top of a submitted form. If you have problems using the interactive online form above, non-interactive off-line printable versions are available on our Forms page. If you want help filling out the form or have problems with the forms please contact Victims Services, by phone, mail or email vs@agd.nsw.gov.au. Further hours counselling
If you have completed the initial hours of approved counselling and would like additional counselling, please complete the following form. For this to be considered, Victims Services also needs a recommendation of further counselling from your Approved Counsellor. For further information please see the page "What happens next"
If you wish to apply for further counselling but don't wish to use the online application above, you can complete and return the following form:  Request for Further Hours of Approved Counselling (pdf 50KB)
Counselling for relatives of homicide victims
If you are a relative of a homicide victim you may be eligible to apply for 20 hours of counselling except:
- where the homicide did not occur in NSW
- it cannot be proved that it was homicide
